4. Accept an offer: When a place becomes available, we will contact you or your nominated contact person.
5. Resident Agreement: Once you have accepted an offer of a place in our Hostel, we will discuss with you a formal Resident
Agreement. The Resident Agreement is legally binding document between you and the Hostel. They set out:

the care and services available

the fees and accommodation costs you will have to pay

how fees and accommodation costs can be paid

your rights and responsibilities.
6. Sign the Resident Agreement if you are agreeable to the terms.

Your health summary

Detail of your regular GP

Medication list

Copy of Medicare and Pension card

Recent hospital discharge summary (if applicable)

Copy of Advanced Care Directives (if applicable)

Copy of Power of Attorney documents (if applicable)

Copy of Medical Substitute Decision Maker (if applicable)
