2. Provide us your filled application form and we will put you down on our waiting list.
3. Accept an offer: When a place becomes available, we will contact you or your nominated contact person.
4. Resident Agreement: Once you have accepted an offer of a place in our Hostel, we will discuss with you a formal Resident
Agreement. The Resident Agreement is legally binding document between you and the Hostel. They set out:

the care and services available
-
the fees and accommodation costs you will have to pay
-
how fees and accommodation costs can be paid
-
your rights and responsibilities.

5. Sign the Resident Agreement if you are agreeable to the terms.

Your health summary
-
Detail of your regular GP
-
Medication list
-
Copy of Medicare and Pension card
-
Recent hospital discharge summary (if applicable)

Copy of Advanced Care Directives (if applicable)

Copy of Power of Attorney documents (if applicable)

Copy of Medical Substitute Decision Maker (if applicable)
